No date or location although what little I've ever been able to find out on this is this style was used for a short time in a couple western states but never went into a full scale production. I think it was from the late 1960s or early/mid 1970s. You'll note it has the swirl of the previous generation and the bottle cap logo of the next without the block border that was very typical for this logo back then. I think of it as a transition bottle that never made the cut. There are no chips or cracks but does have small sratches/wear. It could be cleaned up better.
