It is a modified Omron MY2-02-DC24 relay. It has had two pics removed, and the remaining pins have been bent outward slightly, so it should fit the original holes just fine. I have rewired this relay internally so it matches the pinout of the originals. This is fairly easy to install as long as you know what you're doing and have a decent soldering iron.
